Children´s Choir/Dance Enrollment Forms Fall 2010

Please complete the following information to enroll your child in Choir and/or Dance for the fall of 2010.

Note the start date of Monday September 13, for Choir and Dance. Choir begins afterschool (snack and short play time), and Dance will be from 5-6 p.m.

Please pick up your child by entering the East Lobby, for the safety of all our children.
